WVP HS/EHS Shared Encounter Forms
The Shared Encounter Form (SEF) is the abbreviated paper-pencil version of the WVP. Families, community healthcare workers, and providers can use these forms as a back-up option to using the online WVP tool.
The SEF can be filled out at home, at a community health facility such as Head Start/Early Head Start center, or in a primary practice waiting room. The SEF does not contain all of the questions covered on WVP, but gathers the most important information from the family. The SEF is helpful if the family does not have access to a computer and the internet in order to use the online WVP tool, or if they forgot to bring in the Visit Guide generated by the online tool to review during their child’s visit.
